Vital Records

The Office of Vital Statistics and Registry registers vital events and maintains the following vital records:

  • Birth records from 1923 to present
  • Marriage records from 1941 to present
  • Death records from 1941 to present
  • Domestic partnership records from 2004
  • Civil union records from 2007
  • Certificate of birth resulting in stillbirth from 1969
  • Adoption records - foreign and domestic

Women, Infants & Children (WIC)

WIC is a public health nutrition program that provides wholesome food, nutrition education and community support for eligible women who are pregnant and post-partum with infants and children up to five years old.

New Jersey Department of Health Proposes to Amend the Standards for Licensure of Assisted Living Residences, Comprehensive Personal Care Homes, and Assisted Living Facilities at N.J.A.C. 8:36-1.3 and 2.4 to Establish “Deemed Status” Process and Criteria

The New Jersey Department of Health (Department), with the approval of the Health Care Administration Board, proposes to amend the Standards for Licensure of Assisted Living Residences, Comprehensive Personal Care Homes, and Assisted Living Facilities at N.J.A.C. 8:36-1.3 and 2.4.  The proposed amendments would establish a procedure by which a facility that the Department licenses pursuant to the chapter (facility) could apply for “deemed status.”  “Deemed status” is Department acknowledgment of a facility’s compliance with licensure standards because the facility holds accreditation from a Department-recognized accreditation organization, in place of the Department determining the facility’s compliance status by means of the Department performing a licensure inspection using Department staff.

New Jersey COVID-19 Vaccine Call Center To End Operations

As part of the transition to routine, commercial access to COVID-19 vaccines, the New Jersey COVID-19 Vaccine Call Center is closing operations on March 28, the New Jersey Department of Health (NJDOH) announced today.Residents can continue to call the national COVID-19 Vaccine Hotline at 1-800-232-0233, where assistance is available in English, Spanish, and other languages. Deaf and hard of hearing individuals can call TTY 1-888-720-7489. Persons with disabilities can seek COVID-19 vaccination support through the Disability Information and Access Line at 1-888-677-1199.
